The authors of the 2016 RCPCH Invited Service Review gave evidence at the Thirlwall Inquiry about the terms of reference for that review and how those terms were agreed with Trust executives.

The terms of reference were agreed with the Trust. They were service-level. They were not a mandate to investigate individual clinical incidents, nor to reach a view on whether any specific individual had caused harm. A review of that kind would have required different terms of reference and, almost certainly, different investigators.
