The terms of reference were agreed with the Trust. They were service-level. They were not a mandate to investigate individual clinical incidents, nor to reach a view on whether any specific individual had caused harm. A review of that kind would have required different terms of reference and, almost certainly, different investigators.

And is it correct that the executive team subsequently cited your review as a reason not to contact the police?
