No fatal outcome — no post-mortem (survivor). Baby Q was the surviving triplet of the cohort that included Babies O and P. The clinical record documents a deterioration event with recovery. The three-babies, three-outcomes asymmetry (Babies O and P died; Baby Q survived) is the evidential feature that the jury could not convict on. A theory of the case requiring the same deliberate mechanism to have been applied to all three infants in the same 48-hour window must explain why the third outcome differed from the first two. On a natural-pregnancy-cohort reading, the outcome distribution is not anomalous.
